Esta foi uma prova dramática, uma vez que o líder, o italiano Dorando Pietri, perdeu as forças no final da corrida. Perante um público de 100.000 pessoas ele entrou no Estádio Olímpico para completer a prova e virou a direita, ao invés de virar a esquerda. Antes que as pessoas pudessem apontar a direção correta, Pietri caiu. Médicos e oficiais da prova o ajudaram a ficar de pé novamente, mas esta foi a primeira de mais 4 quedas na volta. Embora Pietri tenha acabado a corrida correndo na direção correta e ter sido o 1º a cruzar a linha de chegada, ele foi desclassificado por ter sido ajudado. A vitória foi para John Hayes, e seu tempo de 2:55:18.4 foi o 1º Recorde Mundial da Maratona.
